This Claim Form is for use in submitting a claim for benefits in the Class Action Settlement Agreement ("Settlement") preliminarily approved by the Marion County, Indiana, Superior Court in the case of Gold Seal Termite and Pest Control Company, et al. v. PrimeTV, L.L.C., et al., Cause No.49D10-0304-CP-0702. Before completing this Claim Form you should read carefully the Notice of Class Action Settlement provided to you with this Form.
In order to be eligible for benefits under the Settlement, you must fully complete, sign and date this Claim Form, and mail it to the Claims Administrator at the address provided below with a postmark no later than September 1, 2004. If your Claim Form is sent to the Claims Administrator with a postmark after September 1, 2004 you will not be eligible for benefits under the Settlement.
